Converting patients to MIRCERA®

Patients currently treated with an erythropoiesis-stimulating agent (ESA) can be converted to intravenous or subcutaneous MIRCERA administered once a month. Select one of three initial doses based on the patient’s current dosing regimen by calculating the micrograms or international units per week for their current ESA (see Table). The first injection should be given at the next scheduled dose of the previously administered ESA.
